Thyroid carcinoma with atypical metastasis to the pituitary gland and unexpected postmortal diagnosis.

Papillary thyroid gland carcinoma is the most common type of malignancy of the endocrine system. Metastases to the pituitary gland have been described as a complication of papillary thyroid cancer in few reported cases since 1965. We report the case of a 68-year-old female patient with a well-differentiated form of thyroid gland cancer. Despite it being the most common malignant cancer of the endocrine system, with its papillary form being one of the two most frequently diagnosed thyroid cancers, the case we present is extremely rare. Sudden cardiac arrest during ventricular fibrillation occurred during hospitalization. Autopsy of the patient revealed papillary carcinoma of the thyroid, follicular variant, with metastasis to the sella turcica, and concomitant sarcoidosis of heart, lung, and mediastinal and hilar lymph nodes. Not only does atypical metastasis make our patient's case most remarkable, but also the postmortem diagnosis of sarcoidosis makes her case particularly unusual.

Recurrent vertigo is common in clinical work. As early as 1979, Slater firstly used benign recurrent vertigo (BRV) to describe that some adult patients present with recurrent vertigo without neurologic or any auditory symptoms (2). In subsequent studies, some scholars found that BRV was closely related to migraine (3,4). But after the International Headache Association and Barany Association proposed vestibular migraine as an independent entity (5), BRV now refers only to recurrent vertigo attacks without etiology. To further explore whether BRV is associated with Meniere's disease or vestibular migraine, we conducted this longitudinal study.

Patellar tendon ruptures make up about 5% of all extensor mechanism injuries. They mainly occur in young, athletic men who have contributing risk factors such as chronic tendinopathy. The aim of this study was to evaluate the return to sport after repair of an acute patellar tendon rupture. Letter to the editor: Clinical features and outcomes of benign recurrent vertigo.

Pan and colleagues describe the clinical features and outcomes of benign recurrent vertigo (BRV) (1). We would like to comment on certain issues. BRV is a common vestibular syndrome; the criteria were established in the past by Rutka (2) and the Committee on Hearing and Equilibrium (3). It is characterized by multiple episodes of vertigo lasting between 5 minutes and 72 hours, without auditory symptoms or headache, so Ménière's disease and vestibular migraine are excluded.
