A case report of Posterior reversible encephalopathy syndrome with spinal cord involvement (PRES-SCI) as an atypical presentation of PRES in children: A case report and review of the literature.

Posterior reversible encephalopathy syndrome (PRES) has a broad spectrum of clinical presentations and radiological features. Diagnosis of PRES is established based on reversible clinical manifestations and sequential neuroimaging findings. Atypical MRI features include hemorrhage, restricted diffusion or contrast enhancement of lesions, and involvement of the temporal and frontal lobes, brainstem, basal ganglia, corpus callosum, cerebellum, and spine. Atypical PRES, with or without spinal cord involvement, is a rare presentation, especially in children. Until 2020, only five cases of PRES with spinal cord involvement (PRES-SCI) were reported in the pediatric population.

Role of Yoga as an Adjunct in the Management of Migraine Headache-Current Status and Future Indications.

Migraine headache is a painful, disabling condition afflicting 7% of the population. The long-term effort of coping with a chronic headache disorder predisposes the individual to other psychiatric illnesses, ischemic cerebrovascular disease as well as medicine overuse headache. The use of nonpharmacological methods to reduce the stress and pain associated with headache can improve the overall quality of life and reduce the burden of the disease. To examine the utility of yoga as an adjunct to pharmacological treatment of migraine headache. The review article is based on the secondary literature collected through the Google Scholar database between the years 2010 and 2020. Several themes were identified regarding the burden of migraine/headache and the need for the integration of yoga into the existing healthcare system. Despite the limitations and the need for greater scientific rigor, there have been consistent reports of the beneficial effects of yoga in the reduction of stress, anxiety, depression, and an enhanced quality of life, as well as better pain management in chronic diseases. Studies on the role of yoga in the treatment of migraine have been few in number. They have consistently shown that yoga can be a valuable adjunct to the existing pharmacological interventions in the management of migraine headache. In recent years, the Indian government has made enormous strides in establishing yoga outreach programs throughout the country. The need of the hour is to integrate evidence-based yoga with the wellness centers and noncommunicable diseases treatment plan. It can help to reduce the burden on the existing health care resources.

Treatment of Chronic Proximal Hamstring Tendinopathy with Surgical Debridement and Suture Anchor Repair.

We evaluated outcomes following surgical debridement and suture anchor repair of chronic proximal hamstring tendinopathy without sciatic nerve decompression. Chart review identified eight patients (one bilateral) who met study requirements. All eight patients were available for follow-up at a mean of 6.7 years and none underwent re-operation on the index hip during the follow-up period. Seven patients completed patient-reported outcome scores. The mean LEFS score was 81.1, and the mean SANE score was 74.9. The mean Marx activity score was 2.8, and the mean Custom Marx score was 23.3. Pain relief was excellent. The mean numeric pain score at rest was 0.6, while the mean numeric pain score with activity was 4.0. Treatment of chronic, recalcitrant proximal hamstring tendinopathy with surgical debridement and suture anchor repair without exploration of the sciatic nerve results in excellent pain relief, good function, and low re-operation risk. Practical application of steroids in palliative therapy.

Összefoglaló. A palliatív ellátás célja a beteg és családja életminőségének javítása egy életet megrövidítő betegség során. A palliatív betegpopuláció 32-80%-a kap szisztémás szteroidot kínzó tünetek enyhítésére. Az alkalmazási irányelvek, a szteroidhasználat a palliatív betegek körében nagyon változó az egyes országok, de az országon belüli ellátók között is. A palliatív betegellátásban szteroidokat – elsősorban szisztémás glükokortikoidokat (dexametazon, betametazon, prednizolon, metilprednizolon) – főleg antiinflammatorikus és a vascularis permeabilitást csökkentő hatásuk miatt alkalmazunk. A palliatív ellátás során glükokortikoid adása számos specifikus indikáció esetén javasolható, mint idegi kompresszió, neuropathiás fájdalom, csontfájdalom és metastasis, májtokfeszülés okozta fájdalom, malignus bélobstrukció, agynyomás-fokozódás, malignus gerincvelő-kompresszió, vena cava superior szindróma, lymphangitis carcinomatosa, nagy légúti obstrukció. Sokszor párhuzamosan jelentkező, nem specifikus tünetek – mint anorexia-cachexia szindróma, hányinger, hányás, fáradtság, gyengeség, nehézlégzés – szintén szükségessé tehetik szteroid adását. A palliatív betegek prognózisát figyelembe véve a szteroidok késői mellékhatásai nem korlátozzák adásukat, az elsődleges terápiás előny felülmúlja a lehetséges kockázatokat. A nemzetközi gyakorlatban a dexametazon a leggyakrabban alkalmazott glükokortikoid, kifejezett gyulladáscsökkentő hatása és kevesebb mineralokortikoid-mellékhatása miatt. Fontos, elengedhetetlen része a biztonságos szteroidterápiának a mellékhatások gondos monitorozása, erről a beteget és a hozzátartozókat is fel kell világosítani. A megfelelő indikációban, dózisban és megfelelő kezelési terv alapján adagolt szteroid hasznos része a palliatív tünetkontrollnak, a beteg életminőség-javításának. Orv Hetil. 2022; 163(8): 294-300. Palliative care aims to improve the quality of life of patients and their families during a life-shortening illness. 32-80% of the palliative patient population receive systemic steroids to relieve torturous symptoms during end-of-life care. Guidelines for steroid use among palliative patients vary widely from country to country, but also within providers. In palliative care, steroids – mainly systemic glucocorticoids (dexamethasone, betamethasone, prednisolone, methylprednisolone) – are used especially for their anti-inflammatory and vascular permeability-reducing effects. Glucocorticoid administration during palliative care is recommended for a number of specific indications, such as neural compression, neuropathic pain, bone pain and metastasis, liver capsule pain, malignant intestinal obstruction, increased cerebral pressure, malignant spinal cord compression, superior vena cava syndrome, carcinomatous lymphangitis, and large airway compression. Often concomitant non-specific symptoms such as anorexia-cachexia syndrome, nausea, vomiting, fatigue, weakness, dyspnoea may also necessitate steroid administration. Considering the prognosis of palliative patients, the late side effects of steroids do not limit their administration, the primary therapeutic benefit outweighs the potential risks. Internationally, dexamethasone is the most commonly used glucocorticoid due to its pronounced anti-inflammatory effect and fewer mineralocorticoid side effects. Careful monitoring of side effects is an important and essential part of safe steroid therapy and should be made clear to the patient and their relatives. Steroids administered in the right indication and dose as well as according to an appropriate treatment plan are useful parts of palliative symptom control and improve patients' quality of life.
