Health problems from pesticide exposure and personal protective measures among women cotton workers in southern Pakistan.

Women are the main cotton pickers in Pakistan even from their childhood, but due to unawareness of pesticide risks and poor knowledge of personal protection opportunities, they typically do not use personal protective measures. Albeit a major agricultural topic in Pakistan, relevant research is limited. The present study illustrates health problems from pesticide exposure during cotton harvest and the use of personal protective measures among 260 female cotton-picking workers from Shaheed Benazirabad District of Sindh Province in southern Pakistan. Skin injury, eye injury, headache, stomachache, and fever were the main health problems due to pesticide exposure according to workers' experience. Medical treatment costs of health problems were more than double (2.48 times higher) than preventive measures costs. More than half of the cotton workers surveyed did not use any protective measure during cotton picking, while 22.3% used a muffler/scarf/cloth for covering their face and 10.8% used gloves. Covering face with muffler/scarf/cloth was higher in young (P < 0.05), married (P < 0.05), and high experienced workers (P < 0.05), while no significant trends were found in the use of gloves. The use of protective measures was positively correlated with education and some experience in cotton picking. Binary logit regression showed that advanced age and high picking experience were positively associated with the use of personal protective measures, while illiteracy, traditional treatment, and medical treatment in case of illness were negatively associated with the use of personal protective measures. Findings shed new light on the topic of personal safety among women cotton pickers in Pakistan. Increasing formal education and implementing training programs for personal protective measures are important to reduce health risk and health cost by pesticides among women workers.

Evaluation of a Hospital Pharmacist Annual Competency Program.

The purpose of this project was to evaluate a pharmacist annual competency evaluation (PACE) program for pharmacists at a single site. The results of this evaluation will be utilized to understand the effectiveness of PACE and provide suggestions for programmatic improvement. The primary outcome of this study was to evaluate the change in pharmacist composite self-efficacy (PSE) scores before and 1 month after the PACE program. The composite score was composed of self-efficacy related to 5 different clinical tasks. The 5 tasks selected were advanced cardiac life support, chemotherapy, dofetilide, patient-controlled analgesia pump, and alteplase (tPA). Secondary outcomes included evaluating each component of the composite score at 1 month, changes in composite score and components at 1 year, and changes in the generalized self-efficacy (GSE) survey at 1 month. Last, themes were identified from a feedback questionnaire after completion of PACE. For the primary endpoint, PSE composite scores increased by 12.6% from pre-PACE to post-PACE at 1 month (79.6 ± 12.2 vs 89.7 ± 5.8; < .001). All components of the composite PSE score showed a statistically significant improvement from baseline to 1 month. At 1 year, tPA was the only clinical task that was statistically different from baseline. GSE scores increased by 11.4% from pre-PACE to post-PACE (31.6 ± 3.6 vs 35.4 ± 2.8). No pharmacists required remediation. The competency assessment program led to improvements in pharmacist self-efficacy in dealing with low-volume, high-risk clinical pharmacy tasks. Future studies could assess various forms of competency assessment and timing of these programs to determine the most effective way to ensure pharmacist competency.

Failure to Provide Adequate Palliative Care May Be Medical Neglect.

Doctors are required to notify Child Protective Services (CPS) if parents do not provide appropriate medical care for their children. But criteria for reporting medical neglect are vague. Which treatments properly fall within the realm of shared decision-making in which parents can decide whether to accept doctors' recommendations? Which treatments are so clearly in the child's interest that it would be neglectful to refuse them? When to report medical neglect concerns to CPS may be controversial. It would seem inhumane to allow a child to suffer because of parental refusal to administer proper analgesia. In this ethics rounds, we present a case of an adolescent with chronic pain who is terminally ill. Her parents were not adherent to recommended analgesia regimens. Her palliative care team had to decide whether to report the case to CPS.

The neuroprotective effects of micronized PEA (PEA-m) formulation on diabetic peripheral neuropathy in mice.

Diabetic peripheral neuropathy (DPN) is a complication of diabetes connected with morbidity and mortality. DPN presents deterioration of peripheral nerves with pain, feebleness, and loss of sensation. Particular medications might display their remedial potential by controlling neuroinflammation. Palmitoylethanolamide (PEA) is an autacoid local injury antagonist distinguished for its neuroprotective, analgesic, and anti-inflammatory properties in numerous experimental models of neuroinflammation. Based on these findings, the goal of this work was to better test the neuroprotective effects of a formulation of micronized PEA (PEA-m) and the probable mechanism of action in a mouse model of DPN induced by streptozotocin (STZ) injection. Diabetic and control animals received PEA-m (10 mg/kg) by oral gavage daily starting 2 wk from STZ injection. After 16 wk, the animals were euthanized, and blood, urine, spinal cord, and sciatic nerve tissues were collected. Our results demonstrated that after diabetes induction, PEA-m was able to reduce mechanical, thermal hyperalgesia, and motor alterations as well as reduce mast cell activation and nerve growth factor expression. In addition, PEA-m decreased neural histologic damage, oxidative and nitrosative stress, cytokine release, angiogenesis, and apoptosis. Moreover, spinal microglia activation (IBA-1), phospho-P38 MAPK, and nuclear factor NF-κB inflammatory pathways were also inhibited. The protective effects of PEA-m could be correlated at least in part to peroxisome proliferator-activated receptor-α activation. In summary, we demonstrated that PEA-m represents a new therapeutic strategy for neuroinflammation pain associated with mixed neuropathies.-Impellizzeri, D., Peritore, A.
