Reported Outcomes in Published Systematic Reviews of Interdisciplinary Pain Treatment: Protocol for a Systematic Overview.

Interdisciplinary pain treatment (IPT) is a complex intervention; its outcomes are very diverse, as are the methodologies for handling those outcomes. This diversity may hamper evidence-based decision making. Presently, there is no gold standard recommendation of how to select reported outcomes in published systematic reviews and meta-analyses to explicitly demonstrate the effectiveness of IPT.

TRPA1 gene variants hurting our feelings.

TRPA1 is a Ca-permeable, non-selective cation channel that is activated by thermal and mechanical stimuli, an amazing variety of potentially noxious chemicals, and by endogenous molecules that signal tissue injury. The expression of this channel in nociceptive neurons and epithelial cells puts it at the first line of defense and makes it a key determinant of adaptive protective behaviors. For the same reasons, TRPA1 is implicated in a wide variety of disease conditions, such as acute, neuropathic, and inflammatory pains, and is postulated to be a target for therapeutic interventions against acquired diseases featuring aberrant sensory functions. The human TRPA1 gene can bare mutations that have been associated with painful conditions, such as the N855S that relates to the rare familial episodic pain syndrome, or others that have been linked to altered chemosensation in humans. Here, we review the current knowledge on this field, re-evaluating some available functional data, and pointing out the aspects that in our opinion require attention in future research. We make emphasis in that, although the availability of the human TRPA1 structure provides a unique opportunity for further developments, far more classical functional studies using electrophysiology and analysis of channel gating are also required to understand the structure-function relationship of this intriguing channel.

Human vestibular perceptual thresholds for pitch tilt are slightly worse than for roll tilt across a range of frequencies.

Vestibular perceptual thresholds measure vestibular sensory and perceptual noise by quantifying how small of a passive self-motion an individual is able to reliably perceive. Vestibular thresholds have clinical and operational relevance, as they are elevated in vestibular migraine patients, and even healthy individuals with higher (i.e., worse) thresholds have degraded balance. Vestibular thresholds have been quantified across a range of frequencies (motion durations) for rotations and translations, with differences identified for different motion directions (e.g., up/down thresholds are higher than those for left/right motions). While roll tilt thresholds have been well quantified, pitch tilt thresholds have not. Here we aim to quantify pitch tilt thresholds across a range of frequencies and test whether they are higher than in those for roll tilt. In ten normal subjects, we found pitch tilt thresholds at 0.15, 0.2, 0.5, and 1 Hz averaged 1.66, 1.61, 0.99, 0.51 degrees, respectively. Using a general linear model, we found subjects' pitch tilt thresholds were slightly, but significantly, higher than their roll tilt thresholds across all frequencies tested. These differences were approximately 10% at 0.15, 0.2, and 1 Hz and 3% at 0.5 Hz. Pitch tilt thresholds exhibited a similar frequency response as in roll tilt (decreasing a higher frequencies). They also had substantial inter-individual variability, which correlated across pitch tilt frequencies and between pitch and roll tilt thresholds. We discuss why pitch tilt thresholds might be higher, including the pitched-up orientation of the utricular plane of the otoliths, compared to previous studies, and discuss functional implications.

Preadmission Cannabis Use Is Positively Correlated With Inpatient Opioid Dose Exposure in Hospitalized Patients With Inflammatory Bowel Diseases.

Opioid use is associated with excess mortality in patients with inflammatory bowel disease (IBD). Recent data have highlighted that inpatient opioid exposure is associated with postdischarge opioid use in this population. It is unknown if preadmission use of cannabis, which is commonly used for symptom relief among patients with IBD, increases the risk for inpatient opioid exposure when patients lack access to cannabis for symptom management. We sought to determine the association between preadmission cannabis use and inpatient opioid exposure while adjusting for relevant confounders.

Relationship between headaches and tinnitus in a Swedish study.

The heterogeneity of tinnitus is likely accounting for the lack of effective treatment approaches. Headaches have been related to tinnitus, yet little is known on how headaches impact tinnitus. We use cross-sectional data from the Swedish Tinnitus Outreach Project to i) evaluate the association between headaches and tinnitus (n = 1,984 cases and 1,661 controls) and ii) investigate the phenotypic characteristics of tinnitus subjects with tinnitus (n = 660) or without (n = 1,879) headaches. In a multivariable logistic regression model, headache was significantly associated with any tinnitus (odds ratio, OR = 2.61) and more so with tinnitus as a big problem (as measured by the tinnitus functional index, TFI ≥ 48; OR = 5.63) or severe tinnitus (using the tinnitus handicap inventory, THI ≥ 58; OR = 4.99). When focusing on subjects with tinnitus, the prevalence of headaches was 26% and reached 40% in subjects with severe tinnitus. A large number of socioeconomic, phenotypic and psychological characteristics differed between headache and non-headache subjects with any tinnitus. With increasing tinnitus severity, fewer differences were found, the major ones being vertigo, neck pain and other pain syndromes, as well as stress and anxiety. Our study suggests that headaches could contribute to tinnitus distress and potentially its severity.
