Trigger of a migraine headache among Thai adolescents smartphone users: a time series study.

The study aims to investigate the effect of modern technology in the trigger of migraine headaches in high school students by using generalized estimating equation analysis. The prospective time series study was conducted in one hundred and forty-five smartphones using high school students in Chiang Mai Province who each completed a headache diary giving a total of 12,969 data entries. The smartphone output power was measured and recorded by a smartphone application. Smartphone use, sleep quality, anxiety, and depression also were assessed. Results revealed that the prevalence of repeated headache be 13.4% and migraine 16.9%. The migraine had the strongest association with handfree device use and internet use, followed by age and anxiety. Furthermore, the strongest effect of smartphone output power in triggering a migraine was found at ≤ 1.79×10-5 and 1.80-1.99×10-5 mW ranges. Meanwhile, Lag_6 of daily smartphone output power exposure produced the migraine effect in a reverse dose-response manner. The smartphone electromagnetic radiation was a primary migraine trigger. The study results led to the recommendation to avoid triggers by use of smartphone talking with hand-free devices to prevent a recurrent migraine.

Stretching Exercise for the Prevention of Postoperative Neck Symptoms Following Thyroid Surgery.

Thyroid surgery is a popular and effective treatment for many thyroid diseases, including thyroid cancer. However, patients who undergo thyroid surgery often experience discomfort symptoms such as stretching, pressing, or choking feelings in the neck, headache, shoulder stiffness, and difficulty in moving the neck or shoulders. These symptoms may persist for long durations. Based on our prospective randomized study published in 2005, we instructed patients who underwent thyroid surgery at Kuma Hospital to perform stretching exercises starting from the day after the surgery. Although we are of the opinion that this management is easy and effective, it might not be well recognized in the world. The purpose of this video is to demonstrate our method of instructing exercises to patients after thyroid surgery and to briefly describe our rehabilitation study.

Analgesic efficacy of paraspinal interfascial plane blocks performed with the use of neurophysiology monitoring for posterior cervical laminectomy surgery: a case series.

Posterior cervical spine surgery often requires large posterior midline incision which can result in poorly controlled postoperative pain, arises from iatrogenic mechanical damage, intraoperative retraction and resection to structures such as bone, ligaments, muscles, intervertebral disks, and zygapophysial joints. Local anesthetics may be utilized for infiltration of the surgical wound; however, their analgesic efficacy has not been studied in this surgical approach. Here we report a case series. Given the potential for targeted sensory dorsal ramus nerve blocks to provide better and extended analgesia, we explored the feasibility of using cervical paraspinal interfascial plane (PIP) blocks in conjunction with neurophysiologic monitoring for postoperative analgesia after posterior cervical laminectomy. Our experience with the cervical paraspinal interfascial plane blocks has revealed that they can be used safely without affecting neurophysiologic monitoring and result in better pain control and reduced opiate use in the postoperative period. Cervical PIP blocks may be useful in controlling pain for posterior cervical laminectomy surgery without compromising neurophysiologic monitoring.

Omentin-1 attenuates glucocorticoid-induced cardiac injury by phosphorylating GSK3β.

Glucocorticoid excess often causes variety of cardiovascular complications, including hypertension, atherosclerosis, and cardiac hypertrophy. To abrogate its cardiac side effects, it is necessary to fully disclose the pathophysiological role of glucocorticoid in cardiac remodelling. Previous clinical and experimental studies have found omentin-1, one of adipokines, exists beneficial effects in cardiovascular diseases, and closely associated with metabolic disorders. However, there is no evidence to address the potential role of omentin-1 in glucocorticoid excess -induced cardiac injuries. To uncover the links, the present study utilized rat model with glucocorticoid-induced cardiac injuries, and clinical patients with abnormal cardiac function. Chronic administration of glucocorticoid excess suppressed the rat serum omentin-1 concentration, which closely correlated with cardiac functional parameters. Intravenous administration of adeno-associated virus encoding omentin-1 upregulated circulating omentin-1 level, and attenuated glucocorticoid excess-induced cardiac hypertrophy, and functional disorders. Overexpression of omentin-1 also improved cardiac mitochondrial function, including reduction of lipid deposits, induction of mitochondrial biogenesis, and enhanced mitochondrial activities. Mechanistically, omentin-1 phosphorylated and activated GSK3β pathway in hearts. From a study of 28 patients with Cushing's syndrome and 23 healthy subjects, plasma level of glucocorticoid was negatively correlated with omentin-1, which exhibiting positively association with cardiac ejection fraction and fractional shortening. Collectively, the present study provided a novel role of omentin-1 in glucocorticoid excess-induced cardiac injuries, and found omentin-1/GSK3β pathway was a potential therapeutic target in combating side effects of glucocorticoid.
