Not my body, not my pain? Pain perception and placebo analgesia in individuals with body integrity dysphoria.

Acute and chronic states of physical pain are inherently linked to our bodily perception. Bodily illusion paradigms have demonstrated that an experimentally induced sense of body disownership can modulate both acute and chronic pain. Insight into the relationship between enduring clinical alterations in body perception and pain is much more limited. The current study examined both pain perception and placebo analgesia in Body Integrity Dysphoria (BID), a clinical model of long-term alterations of bodily disownership: in its most commonly studied variant, people feel like a part of their body does not belong to them, leading to a desire for amputation of a physically healthy limb. Heat stimulations were applied before and after a placebo intervention (sham analgesic cream) to the desired and the undesired leg of 19 patients with BID with a unilateral leg amputation desire. Pain perception was assessed using pain thresholds, and ratings for pain intensity and pain unpleasantness. Results show that pain perception and placebo efficacy were lower for the undesired than for the desired leg, demonstrating a potential link between a clinical disorder of body ownership, pain perception, and placebo analgesia.

Dysfunctional sleep beliefs and behaviours: Psychological factors in sleep-related headaches.

Sleep problems are amongst the most common triggers of migraine and non-migraine primary headache. Despite a majority of sleep problems being psychological in nature, there is a paucity of quantitative research on the psychological factors involved in sleep-related headaches. This is the first study to examine the link between maladaptive sleep beliefs and headaches. 542 participants completed an online battery measuring headache, sleep, and psychological distress. Avoidance of sleep-related headache triggers was associated with more headaches, as mediated by increased sensitivity to the triggers. Sleep quality, sleep beliefs, and sleep behaviours all significantly (p < .001) correlated with headache frequency and disability, with effect sizes ranging from small (r = 0.16) to medium (r = 0.37). Mediation models testing the effect of sleep beliefs on headaches via sleep quality (covaried by sleep behaviours) accounted for 13% and 14% of variance in migraine and non-migraine headaches, respectively (p < .001). Avoidance of sleep-related headache triggers was shown to be associated with greater headache frequency (via increased trigger sensitivity). It is posited that maladaptive sleep beliefs and behaviours may increase headache activity by causing poor sleep quality, leading to dysregulation in brain regions shared between sleep and headaches.

Taking the pulse of nations: A biometric measure of well-being.

A growing literature identifies associations between subjective and biometric indicators of wellbeing. These associations, together with the ability of subjective wellbeing metrics to predict health and behavioral outcomes, have spawned increasing interest in wellbeing as an important concept in its own right. However, some social scientists continue to question the usefulness of wellbeing metrics. We contribute to this literature in three ways. First, we introduce a biometric measure of wellbeing – pulse – that hs been little used. Using nationally representative data on 165,000 individuals from the Health Survey for England and Scottish Health Surveys we show that its correlates are similar in a number of ways to those for happiness, and that it is highly correlated with wellbeing metrics, as well as self-assessed health. Second, we examine the determinants of pulse rates in mid-life (age 42) among the 9000 members of the National Child Development Study, a birth cohort born in a single week in 1958 in Britain. Third, we track the impact of pulse measured in mid-life (age 42) on health and labor market outcomes at age 50 in 2008 and age 55 in 2013. The probability of working at age 55 is negatively impacted by pulse rate a decade earlier. The pulse rate has an impact over and above chronic pain measured at age 42. General health at 55 is lower the higher the pulse rate at age 42, while those with higher pulse rates at 42 also express lower life satisfaction and more pessimism about the future at age 50. Taken together, these results suggest social scientists can learn a great deal by adding pulse rates to the metrics they use when evaluating people's wellbeing.

Excretion, Metabolism, and Tissue Distribution of () in Pigs.

() is a toxic flowering plant in the family Loganiaceae used to treat skin diseases, neuralgia and acute pain. The high toxicity of restricts its development and clinical applications, but in veterinary applications, has been fed to pigs as a feed additive without poisoning. However, until now, the in vivo processes of the multiple components of have not been studied. This study investigates the excretion, metabolism and tissue distribution of the multiple components of after feeding it to pigs in medicated feed. Pigs were fed 2% powder in feed for 45 days. The plasma, urine, bile, feces and tissues (heart, liver, lung, spleen, brain, spinal cord, adrenal gland, testis, thigh muscle, abdominal muscle and back muscle) were collected 6 h after the last feeding and analyzed using high-performance liquid chromatography coupled to quadrupole time-of-flight mass spectrometry. Five natural products in plasma, twelve natural products and five metabolites in urine, and three natural products in feces were characterized, suggesting that multiple components from were excreted in the urine. However, ten natural products and four metabolites were detected in bile samples, which suggested that is involved in enterohepatic circulation in pigs. A total of seven of these metabolites were characterized, and four metabolites were glucuronidated metabolites. Ten natural products and six metabolites were detected in the tissues, which indicates that is widely distributed in tissues and can cross the blood-brain barrier. Among the characterized compounds, a highly toxic gelsedine-type alkaloid from was the main compound detected in all biological samples. This is the first study of the excretion, metabolism and tissue distribution of multiple components from in pigs. These data can provide an important reference to explain the efficacy and toxicity of . Additionally, the results of the tissue distribution of are of great value for further residue depletion studies and safety evaluations of products of animals fed .

Friedelin Alleviates the Pathogenesis of Collagenase-Induced Tendinopathy in Mice by Promoting the Selective Autophagic Degradation of p65.

With the development of an aging population, tendinopathy has become a common musculoskeletal disease in the elderly with a high recurrence rate and no curative treatment. The inflammation mediated by NF-κB signaling plays an important role in tendon senescence and degeneration. Friedelin (FR) is a triterpenoid derived from green plants, which has a variety of pharmacological functions, such as analgesia, anti-inflammation, antioxidation, and anti-tumor functions. However, the role and mechanism of FR in tendinopathy are unclear. Here, we found that FR improved the mechanical strength of the Achilles tendon, restored the orderly arrangement of collagen fibers, reduced inflammatory cell infiltration, and promoted tenogenesis, thereby blocking the progression of tendinopathy. Mechanistically, FR promoted the autophagic degradation of p65 by enhancing the interaction between p62 and p65 and effectively inhibited the activation of the NF-κB pathway, thus alleviating the inflammatory response of tenocytes. In addition, FR recruited E3 ubiquitin enzyme RNF182 to increase the K48-linked ubiquitination of p65 and promoted p62-mediated autophagic degradation. Furthermore, blocking ubiquitination reversed the degradation of p65 by FR. Therefore, these findings identify the new pharmacological mechanism of the anti-inflammatory effect of FR and provide a new candidate drug for the treatment of tendinopathy.

Degenerative Meniscus in Knee Osteoarthritis: From Pathology to Treatment.

Knee osteoarthritis is a common degenerative joint disease characterized by chronic knee pain and disability in daily living. The lesion can involve the cartilage as well as the synovium, bone, ligaments, and meniscus, indicating a complicated pathology for knee osteoarthritis. The association with the meniscus has recently attracted much attention. Meniscal tears can initiate and progress knee osteoarthritis, with deleterious effects on the important roles of the meniscus in load distribution, shock absorption, and stability of the knee joint. Degenerative meniscus lesions are commonly observed in elderly people, but they have less impact on the prognosis of osteoarthritis. However, they are often accompanied by meniscal extrusion, which substantially decreases the hoop function of the meniscus and increases the risk of knee osteoarthritis. When surgical treatment is necessary, meniscal tears should be repaired to the greatest extent possible to preserve meniscus function. Long-term studies show better clinical outcomes and less degenerative osteoarthritis changes following meniscal repair than following partial meniscectomy. For meniscal extrusion repair, centralization techniques have been proposed that involve suturing the meniscus-capsule complex to the edge of the tibial plateau. Advancements in orthobiologics, such as platelet-rich plasma or stem cell therapy, have the potential to prevent the initiation or progression of osteoarthritis.
