Neurological monitoring and sedation protocols in the Liver Intensive Care Unit.

Patients with liver disease often have alteration of neurological status which requires admission to an intensive care unit. Patients with acute liver failure (ALF), acute-on-chronic liver failure (ACLF) and rarely cirrhosis are at risk of cerebral edema. These patients require prompt assessment of neurological status including assessment of intra-cranial pressure (ICP) and monitoring metabolic parameters like arterial/venous ammonia levels, serum creatinine and serum electrolytes so that timely specific therapy for raised ICP can be instituted to prevent permanent neurological dysfunction. The overall aims of neuromonitoring and sedation protocols in a liver intensive care unit are to identify the level of multifactorial metabolic encephalopathy, individualize sedation and analgesia requirements for patients on mechanical ventilation, institute specific therapy to correct the neurological insult in ALF and ACLF, provide clear physiological data for guided therapy of drugs like muscle relaxants, antiepileptics, and cerebral edema reducing agents, and assist with overall prognostication. In this review article we will outline the clinical scenarios related to liver disease requiring intensive care and neuromonitoring, current techniques of neurological assessment, sedation protocols and point of care tests which enable the treating physician and intensivist guide therapy for raised ICP.

Effectiveness of different protocols to reduce postoperative pain following orthognathic surgery: A systematic review and meta-analysis.

Pain management in orthognathic surgery is essential to enhance recovery, reduce hospital stay, and improve the whole experience of the patient. The aim of this systematic review was to evaluate current evidence on pain management in orthognathic surgery. A systematic review of the literature was performed following PRISMA guidelines, and PubMed, EMBASE, and the Cochrane Controlled Trials Registry were searched to retrieve randomised clinical trials (RCTs) published until July 2020. RCTs that compared different pre-emptive analgesia and low-level laser therapy (LLLT) with placebo after orthognathic surgery were included. Outcome variables were pain scores and duration of surgery. The quality of evidence was rated according to Cochrane's tool for assessing risk of bias. Standardised mean difference (SMD) or mean difference (MD) was used to analyse continuous data. There was significant pain reduction within the first 48 hours after pre-emptive analgesia (very low quality evidence, SMD: -1.329; confidence interval (CI): -2.030 to -0.628; p = 0.001) and LLLT (very low quality evidence, SMD: -0.690; CI: -1.172 to -0.207; p = 0.005) versus placebo. Evidence to support pre-emptive analgesia or LLLT effectively reducing pain scores within the first postoperative 48 hours after orthognathic surgery when compared with placebo, was of low quality.

Dry Needling in Physical Therapy Treatment of Chronic Neck Pain: Systematic Review.

Chronic Neck Pain (CNP) is one of the main causes of disability worldwide, and it is necessary to promote new strategies of therapeutic approach in the treatment of chronic pain. Dry needling (DN) is defined as an invasive physiotherapy technique used in the treatment of neuromusculoskeletal disorders. The purpose of this review was to assess the effectiveness of invasive techniques in treatment of CNP. The search focused on randomized clinical trials, and according to the selection criteria, eight studies were obtained. In conclusion, DN can be an effective treatment option for CNP, positive outcomes were achieved in the short-term and in the follow-up performed between three and six months, and this technique may offer better outcomes than a placebo intervention based on the application of simulated DN.

The cherry silverberry ( Thunb.) is a lesser-known plant species with high nutritional and therapeutic potential. Cherry silverberry contains numerous biologically active compounds. The cherry silverberry is a shrub growing up to 3 m. Its drupe-like fruit is ellipsoidal, up to 1 cm long, and set on stems. It is red in color, juicy, and sour, and its taste resembles that of red currants. According to the literature, cherry silverberry fruit contains carbohydrates, organic acids, and amino acids, as well as vitamin C, in addition to biominerals, polyphenols, flavonoids, carotenoids, chlorophylls, and tocopherols, which contribute to its high nutritional value. New biotypes of cherry silverberry cultivated in Poland can be used for the production of functional foods and direct consumption. In China, the cherry silverberry, known as goumi, has been used as a medicinal plant and a natural remedy for cough, diarrhea, itch, foul sores, and, even, cancer. This review article summarizes the scant research findings on the nutritional and therapeutic benefits of cherry silverberry.
