Effects of exercise/physical activity on fear of movement in people with spine-related pain: protocol for a systematic review and meta-analysis.

Low back pain and neck pain are leading causes of disability. Although several studies have examined the effect of exercise on fear of movement in people with spine-related pain, the overall evidence supporting the beneficial effect of different forms of exercise on fear of movement remains unknown. This systematic review will determine the strength of evidence for the effect of exercise/physical activity on fear of movement in people with non-specific spine-related pain.

Fascial Innervation: A Systematic Review of the Literature.

Currently, myofascial pain has become one of the main problems in healthcare systems. Research into its causes and the structures related to it may help to improve its management. Until some years ago, all the studies were focused on muscle alterations, as trigger points, but recently, fasciae are starting to be considered a new, possible source of pain. This systematic review has been conducted for the purpose of analyze the current evidence of the muscular/deep fasciae innervation from a histological and/or immunohistochemical point of view. A literature search published between 2000 and 2021 was made in PubMed and Google Scholar. Search terms included a combination of fascia, innervation, immunohistochemical, and different immunohistochemical markers. Of the 23 total studies included in the review, five studies were performed in rats, four in mice, two in horses, ten in humans, and two in both humans and rats. There were a great variety of immunohistochemical markers used to detect the innervation of the fasciae; the most used were Protein Gene Marker 9.5 (used in twelve studies), Calcitonin Gene-Related Peptide (ten studies), S100 (ten studies), substance P (seven studies), and tyrosine hydroxylase (six studies). Various areas have been studied, with the thoracolumbar fascia being the most observed. Besides, the papers highlighted diversity in the density and type of innervation in the various fasciae, going from free nerve endings to Pacini and Ruffini corpuscles. Finally, it has been observed that the innervation is increased in the pathological fasciae. From this review, it is evident that fasciae are well innerved, their innervation have a particular distribution and precise localization and is composed especially by proprioceptors and nociceptors, the latter being more numerous in pathological situations. This could contribute to a better comprehension and management of pain.

The Involvement of Alarmins in the Pathogenesis of Sjögren’s Syndrome.

Sjögren's syndrome (SS) is a chronic autoimmune disease that affects exocrine glands, primarily the salivary and lachrymal glands. It is characterized by lymphoplasmacytic infiltration of the glandular tissues, ultimately leading to their dysfunction and destruction. Besides classic dry eyes and dry mouth defined as sicca syndrome, patients affected by the disease also typically display symptoms such as fatigue, pain and in more than 50% of cases, systemic manifestations such as arthritis, interstitial lung involvement, neurological involvement and an increased risk of lymphoma. The pathophysiological mechanisms underlying SS still remain elusive. The crucial role of innate immunity has been advocated in recent years regarding the pathogenesis of pSS, especially in the initiation and progression toward autoimmunity. Alarmins are endogenous molecules that belong to the large family of damage associated molecular pattern (DAMP). Alarmins are rapidly released, ensuing cell injury and interacting with pattern recognition receptors (PRR) such as toll-like receptors (TLR) to recruit and activate cells of the innate immune system and to promote adaptive immunity responses. This review highlights the current knowledge of various alarmins and their role in the pathogenesis of pSS.

Disclosing Critical Voice Features for Discriminating between Depression and Insomnia-A Preliminary Study for Developing a Quantitative Method.

Depression and insomnia are highly related-insomnia is a common symptom among depression patients, and insomnia can result in depression. Although depression patients and insomnia patients should be treated with different approaches, the lack of practical biological markers makes it difficult to discriminate between depression and insomnia effectively. This study aimed to disclose critical vocal features for discriminating between depression and insomnia. Four groups of patients, comprising six severe-depression patients, four moderate-depression patients, ten insomnia patients, and four patients with chronic pain disorder (CPD) participated in this preliminary study, which aimed to record their speaking voices. An open-source software, openSMILE, was applied to extract 384 voice features. Analysis of variance was used to analyze the effects of the four patient statuses on these voice features. statistical analyses showed significant relationships between patient status and voice features. Patients with severe depression, moderate depression, insomnia, and CPD reacted differently to certain voice features. Critical voice features were reported based on these statistical relationships. This preliminary study shows the potential in developing discriminating models of depression and insomnia using voice features. Future studies should recruit an adequate number of patients to confirm these voice features and increase the number of data for developing a quantitative method.

Cerebral Venous Thrombosis after SARS-CoV-2 Infection and Pfizer-BioNTech Vaccination against COVID-19.

In the last 3 years, COVID-19 pandemic has produced great impacts on global population in terms of health and social costs. Pneumonia represents only one of several pathologies associated to COVID-19 disease. Among these, the cerebral venous thrombosis (CVT), constitutes an important cause of stroke. Here, we report a case of CVT diagnosed approximately 2 weeks after first dose of Pfizer-BioNTech vaccination, in a patient affected by COVID-19 few months earlier. He presented with headache and severe asthenia. The laboratory tests put in evidence thrombocytopenia and D-dimer elevation. A brain magnetic resonance imaging (MRI) and a computed tomography (CT) demonstrated hemorrhagic and ischemic phenomena on the right ventral thalamic nuclei, left thalamus, hippocampal and parahippocampal regions and the splenium of the corpus callosum. The study revealed a poorly opacified vein of Galeno and straight sinus. Heparin administration improved his clinical status; platelets values also arose over time.
