Unique Presentation and Novel Surgical Approach to a Transcribriform Penetrating Head Injury Caused by a Nail Gun.

A penetrating head injury caused by a nail gun is an infrequent clinically diverse condition that varies in severity by the neurovascular structures involved. The authors present the case of a patient whose frontal lobe was pierced by a nail that entered via a transnasal transcribriform trajectory without causing vascular injury or intracranial hemorrhage; the man was unaware of the nail's presence and presented with headache five days after the incident. The nail was extracted using a bifrontal craniotomy for direct visualization and for defect repair of the skull base combined with endoscopic endonasal extraction of the nail.

Seizure-Induced Spinal Fractures: A Systematic Review.

Patients with epilepsy have a higher risk of skeletal injuries, and some of these fractures occur in the spine during a seizure. Seizure-induced spinal fractures (SISF) are unusual injuries that typically occur in the thoracolumbar spine. Because these skeletal injuries are not well described in the literature, this article aims to analyze the characteristics of this uncommon clinical entity.

Gastrointestinal tuberculosis presenting as acute abdomen: a case report.

Intestinal tuberculosis (ITB) represents an important diagnostic challenge in the clinical setting, as it generally can occur as a chronic condition often mimicking other diseases such as Crohn's disease (CD), and can present itself with acute onset, which can be life-threatening. A 29-years-old Chinese woman coming to ER with abdominal pain, fever and weight loss. Computed tomography and colonoscopy images were not diagnostic. Despite medical therapy, the patient progressively worsened developing sepsis requiring emergency surgery. Pathological and microbiological examination of the colon both pointed towards gastrointestinal tuberculosis involvement. Although ITB is generally a chronic-wasting condition, it can also occur as acute abdomen representing an absolute surgical emergency. Although caseation and necrosis in granulomas can be used for diagnosis of ITB, preoperative diagnosis is still challenging. Endoscopic biopsies targeted to ulcerous lesions could be an essential diagnostic tool, contrary to those targeted to the ulcers' edges as performed in CD.

[A case of Coexistent Persistent Trigeminal and Hypoglossal Arteries Manifested with Neurovascular Compression Syndrome by Hypertension].

A 70-year-old woman visited our hospital with hypertension, diplopia, and right orbital pain. Neurological examination revealed right ophthalmoplegia. CT angiography and MRI identified a right persistent trigeminal artery (PTA), right persistent hypoglossal artery, and bovine aortic arch. The right internal carotid artery (ICA) was displaced laterally in the cavernous sinus due to the bifurcation of the PTA. Compression of the right oculomotor nerve, right trochlear nerve, and first division of the right trigeminal nerve by the elongated right ICA was noted and considered a potential cause of the ophthalmoplegia and orbital pain. Symptoms improved with normalization of blood pressure. During embryonic development, the right posterior communicating artery and bilateral vertebral arteries were aplastic or hypoplastic, which suggests that these carotid-basilar anastomoses may have remained as supply routes to provide sufficient blood flow to the posterior cerebral circulation. This is an extremely rare case of embryological implications manifested with neurovascular compression syndrome. (Received 6 January, 2022; Accepted 17 February, 2022; Published 1 June, 2022).

Activity and safety of topical pimecrolimus in patients with early stage mycosis fungoides (PimTo-MF): a single-arm, multicentre, phase 2 trial.

The calcineurin pathway is often activated in mycosis fungoides. We aimed to assess the activity and safety of topical pimecrolimus, a calcineurin inhibitor, in patients with early mycosis fungoides.

The updated pre-test probability model of the 2019 ESC guidelines improves prediction of obstructive coronary artery disease.

The 2019 ESC guidelines on chronic coronary syndromes updated the method for estimating the pre-test probability (PTP) of obstructive coronary artery disease (CAD). We aimed to compare the performance of the new PTP method against the 2013 prediction model in patients with stable chest pain undergoing coronary computed tomography angiography (CCTA) for suspected CAD.
