A Case of Recurrent Epiploic Appendagitis Treated With Conservative Management.

Epiploic appendagitis is a rare and commonly misdiagnosed cause of acute abdominal pain. Treatment for the initial presentation of epiploic appendagitis is conservative management with anti-inflammatory medications. There is no consensual treatment algorithm for recurrent appendagitis as some studies recommend conservative management and others recommend surgical excision. This case report will cover a patient who presented with recurrent epiploic appendagitis and was treated with conservative management.

Real world experience with minimally invasive wireless percutaneous neuromodulation in a tertiary care centre.

Wireless percutaneous nerve stimulation (PNS) for chronic pain is rapidly evolving in the ever expanding neuromodulation paradigm. Safety and lower risks with a potential of long-term analgesia cannot be over emphasised especially with the ongoing opioid pandemic. PNS with an implanted pulse generator (IPG) has also been shown to provide good benefit, without often unpleasant widespread paraesthesia from conventional spinal cord stimulators.

Outcomes of a Modified Arthroscopic-assisted Reconstruction Technique for Lateral Ankle Instability.

 The present study assesses the results of a minimally invasive surgical technique for acute and chronic ankle instability management.  The present case series study retrospectively evaluated 40 patients undergoing arthroscopic-assisted percutaneous ankle ligament reconstruction from 2013 to 2019.  The present study included 17 males and 23 females with an average age of 38.3 years old. Postintervention follow-up using American Orthopaedic Foot and Ankle Society (AOFAS) Ankle-Hindfoot scores identified improvement of > 30 points in function and pain control. The most frequently occurring associated injuries were osteochondral (35%). No patient required reintervention or had infection during follow-up.  The technique in the present study is easy and achieves satisfactory results for function and pain control. IV.

Hughes abdominal closure versus standard mass closure to reduce incisional hernias following surgery for colorectal cancer: the HART RCT.

Incisional hernias can cause chronic pain and complications and affect quality of life. Surgical repair requires health-care resources and has a significant associated failure rate. A prospective, multicentre, single-blinded randomised controlled trial was conducted to investigate the clinical effectiveness and cost-effectiveness of the Hughes abdominal closure method compared with standard mass closure following surgery for colorectal cancer. The study randomised, in a 1 : 1 ratio, 802 adult patients (aged ≥ 18 years) undergoing surgical resection for colorectal cancer from 28 surgical departments in UK centres.
