Effectiveness of Platelet-Rich Plasma in Anterior Cruciate Ligament Reconstruction: A Systematic Review of Randomized Controlled Trials.

This study aimed to identify the effectiveness of platelet-rich plasma (PRP) for patients operated with anterior cruciate ligament reconstruction (ACLR). Databases of PubMed, Embase, and CENTRAL were independently retrieved by two authors, for identifying the eligible randomized controlled trials (RCTs) comparing the clinical and imaging outcomes of ACL reconstructed patients augmented with or without PRP. The Cochrane Collaboration tool was utilized to assess the risk of bias of the included trials. We qualitatively synthesized the outcomes include the image evaluations on the healing of bone tunnels, graft remodeling, donor site healing and tunnel widening, and clinical evaluations on knee stability and function, pain symptom by visual analogue scale (VAS), inflammatory parameters and so on. A total of 16 RCTs, including 1025 patients, were included for eligibility. Generally, the included studies were of low risk of bias, but the conducting of allocation concealment was not clearly described in many studies. Three imaging techniques, including MRI, CT and ultrasound, were selected in these trials. Significant improvement on graft remodeling, bone tunnel healing, harvest site healing and bone tunnel diameters were demonstrated in one of five (20.0%), three of five (60.0%), two of four (50.0%) and one of five (20.0%) studies respectively, for PRP group. Various clinical outcomes, such as IKDC score, Lysholm score, Tegner score, knee anteroposterior and rotational laxity, range of motion and VAS, could not be improved with PRP application. The PRP is associated with very limited role in improving knee outcomes following ACLR, and there is no indication for PRP procedures in ACLR at this stage.

Antioxidant and Anti-Inflammatory Activity of Ethanol Extract Stem of A.D. Poulsen.

<b>Background and Objective:</b> <i>Etlingera rubroloba</i> A.D. Poulsen (<i>E. rubroloba</i>) is a plant endemic to South-East Sulawesi which is used empirically by local people as a pain reliever for joints and to increase endurance after typhoid fever. This study aimed to determine the antioxidant and anti-inflammatory activity of the ethanol extract stem of <i>E. rubroloba</i>. <b>Materials and Methods:</b> The stem of the ethanol extract of <i>E. rubroloba</i> with evaluated for antioxidants using the FRAP method and anti-inflammatory activity with the parameter level of Tumor Necrosis Factor-Alpha (TNF-α) <i>in vivo</i> by using 6 groups of rats, namely the normal, the negative (Na-CMC 0.5%), the positive control (Diclofenac sodium) and the ethanol extract group at doses of 200, 300 and 400 mg kg<sup>1</sup> b.wt. <b>Results:</b> The results of this study indicated that the antioxidant activity value of the IC<sub>50</sub> ethanol extract was 12.720±0.12 μg mL<sup>1</sup> and ascorbic acid (vitamin C) as a standard control was 3.14±0.12 μg mL<sup>1</sup>. The TNF- α normal group (7.83 pg mL<sup>1</sup>), negative control (250.92 pg mL<sup>1</sup>), positive control (123.66 pg mL<sup>1</sup>), treatment group dose 200 (192.20 pg mL<sup>1</sup>), 300 (97, 95 pg mL<sup>1</sup>) and 400 mg kg<sup>1</sup> b.wt. (28.78 pg mL<sup>1</sup>). <b>Conclusion:</b> This study concluded that the ethanol extract of the stem of <i>E. rubroloba</i> has a very strong antioxidant activity and is an anti-inflammatory which is the best in reducing levels of TNF-α at a dose of 400 mg kg<sup>1</sup> b.wt.

Recent advances in the management of hemiplegic shoulder pain.

Stroke is a debilitating neurological disorder with hemiplegia as most common presentation. Hemiplegic shoulder pain (HSP) affects a large number of stroke survivors and is associated with significant morbidity and low quality of life (QoL). The etiology is multifactorial and therefore the management is multipronged. Traditionally, oral analgesics have been advised with physical therapy and intra-articular steroid injections. This narrative review discusses emerging treatment strategies for HSP. It focuses on four key new treatments; electric stimulation, the use of robotics, intraarticular injections of novel anti-inflammatory agents, and pulsed radiofrequency treatment. Multiple studies have assessed the efficacy of these techniques and have found the efficacy and side effect profile to be comparable or superior to current management strategies for HSP. Integrating these interventions in the multidisciplinary rehabilitation programmes for stroke patients, can improve the management of HSP and reduce stroke related morbidity and disability.

Practical Measurement of Changes in Leg Length Discrepancy After a Myofascial Release on the Thoracolumbar Fascia in Patients With Acute Low Back Pain: A Pilot Study.

Background Recent work has examined an association between leg length discrepancy (LLD) and low back pain (LBP). Myofascial release (MFR) techniques are thought to be frequently applied in the treatment of chronic and acute LBP. The purpose of this study was to evaluate a practical measure of LLD and the feasibility of an MFR technique in a randomised controlled trial (RCT). Methods In 12 subjects (seven women and five men) with acute LBP and LLD greater than 3 mm, an MFR technique was performed on the thoracolumbar fascia. At the baseline, after the intervention, and at follow-up, LLD was measured using a cross-line laser and finger-to-floor distance, and the pain was measured with a visual analogue scale (VAS). Patients completed a questionnaire after follow-up to assess the acceptability of the study procedure. The therapist evaluated the methods in terms of their feasibility. Results LLD measurement and MFR treatment required little time and few resources. Participants agreed to the study procedure with moderate to high acceptance. The LLD decreased by 5 mm after treatment and by 4 mm at follow-up. The VAS showed a reduction in pain of 17.50 mm at follow-up but not immediately after treatment. Conclusion The measurement of LLD is applicable in daily osteopathic practice, but it cannot be assumed to be a valid method for an RCT. Validated methods such as video raster stereography are, therefore, recommended. Comprehensive RCTs to study the effects of MFR intervention on leg length are feasible.

Post-COVID-19 Vaccine Limbic Encephalitis: A Case Report.

Limbic encephalitis (LE) diagnosis can be challenging due to its broad spectrum of clinical presentation and variety of causes. The most commonly known causes include paraneoplastic and autoimmune, but they can also occur post-vaccine. Since 2020, many people worldwide have received the coronavirus disease 2019 (COVID-19) vaccine after FDA approval. Mild self-limited neurological adverse reactions, including headache and dizziness, were reported post-vaccine. However, emerging few neurological severe events, including encephalitis, have also been reported. Herein, we present a case of a middle-aged female who presented with seizures after two days of receiving the second dose of the Moderna COVID-19 vaccine. A diagnosis of limbic encephalitis was made based on head MRI findings. It was treated with immunosuppressive agents and responded well with no additional neurological sequelae. This case is unique as it highlights a possible association between limbic encephalitis and the COVID-19 vaccine.

Carotid Body Tumor: Our Experience with 42 Patients and a Literature Review.

To review characteristics, signs, diagnostic methods, and surgical complications in patients with carotid body tumor, in addition to a literature review. This was a cross-sectional study carried out on 42 patients with carotid body tumors referred to Al-Zahra hospital of Isfahan from January 1, 2016, to December 31, 2019. The studied characteristics included age at the onset of symptoms, gender, symptoms, duration of symptomatic status before diagnosis, diagnostic methods, pathological outcome, tumor stage, and surgical complications. The collected data were analyzed using SPSS 21. In this study, 34 patients (80.96%) were female. The mean age at the onset of symptoms was 54.41 years. The mean duration of symptoms before diagnosis was 12.55 months. There was no significant difference between genders for the age and duration of symptoms before diagnosis ( > 0.05). Symptoms were mainly painless mass, hoarseness, coughing, earache, tinnitus, and headache. After surgery, two cases of permanent cranial nerve injuries were found. Thirty-nine patients (93%) were diagnosed by ultrasound imaging and CT scan. We had one case of arterial repair. There was one malignant and one functional tumor. According to the results, carotid body tumor is more common in females especially on the right side. Bilateral tumors are prevalent in females. This mass should be removed by surgery at the time of diagnosis in cases where the patient is not at high risk for surgery due to comorbidities, and the risk of complications following surgery is low.

Cutting the cord? Parenting emerging adults with chronic pain.

The role of parent factors, such as distress and protective behaviors, on pain and functional outcomes of emerging adults living with chronic pain has been largely unexplored. The effects of helicopter parenting and developmental changes occurring during this transition period between adolescence and adulthood (commonly defined as the ages between 18 and 30 years) may exacerbate the pain experience and have the potential to influence chronic pain management. Clinical practice, with an additional focus on supporting the parent(s), may aid in meeting the needs of this population. In this paper, we review the available literature on (a) the socio-cultural shift in parenting over the past decade with a focus on helicopter parenting; (b) the impact of this parenting style on the pain experience and outcomes of emerging adults living with chronic pain; (c) provide recommendations for chronic pain management with a focus on the parent-emerging adult dyad; and (d) conclude with future research recommendations. This narrative review is the first to consider the impacts and outcomes of helicopter parenting on emerging adults with chronic pain.
