The Use of Low-Dose Naltrexone for Chronic Pain.

The use of low-dose naltrexone for the treatment of chronic pain is novel because it is a nonopioid alternative. Oral naltrexone in a dosage range from 1 mg to 4.5 mg is referred to as low-dose naltrexone. Low-dose naltrexone use is "off label" and has been used successfully to manage chronic pain, autoimmune disorders, and dermatologic conditions. Low-dose naltrexone could be a viable treatment option for chronic pain because other agents for chronic pain, such as nonsteroidal agents, have adverse effects of gastrointestinal bleeding, renal injury, and increase a patient's risk of myocardial infarction or stroke. Additionally, low-dose naltrexone has minimal adverse effects, no drug-drug interactions, and is relatively inexpensive compared with other options for chronic pain.

Pyloric Obstruction Caused by an Inflammatory Fibroid Polyp.

This is a case report of a 57-year-old woman who presented with abdominal pain and vomiting over a period of two months. Upper gastrointestinal endoscopies and biopsies were inconclusive, while abdomen computed tomography (CT) scan revealed a large mass arising from the pyloric antrum measuring about 6 × 4.8 cm imitating gastrointestinal stromal tumor (GIST). The patient underwent a laparotomy, and the tumor was totally resected with well-defined borders. The histopathological analysis revealed the mass to be an inflammatory fibroid polyp (IFP).

CHIVA to treat saphenous vein insufficiency in chronic venous disease: characteristics and results.

There is considerable debate in the literature with relation to the best method to treat patients with chronic venous disease (CVD). CHIVA is an office-based treatment for varicose veins performed under local anesthesia. The aim of the technique is to lower transmural pressure in the superficial venous system and avoid destruction of veins. Recurrence of varicosities, nerve damage, bruising and suboptimal aesthetic results are common to all treatments for the disease. This paper evaluates and discusses the characteristics and results of the CHIVA technique. We conclude that CHIVA is a viable alternative to common procedures that is associated with less bruising, nerve damage, and recurrence than stripping saphenectomy. The main advantages are preservation of the saphenous vein, local anesthesia, low recurrence rates, low cost, low pain, and no nerve damage. The major disadvantages are the learning curve and the need to train the team in venous hemodynamics.

Epidermolytic hyperkeratosis: clinical update.

Epidermolytic hyperkeratosis (EHK), earlier termed as bullous congenital ichthyosiform erythroderma is a skin disorder characterized as an autosomal dominant and rare disorder which has been observed to affect 1 in over 200,000 infants as a consequence of a significant mutation in the genes responsible for the keratin proteins, mostly keratin 1 and 10. The features present at birth include erythema and blistering. In adults, the hallmarks include hyperkeratosis, erosions, and blisters. The major symptoms including xerosis, pruritus, and painful fissuring lead not only to cosmetic problems but also stress, inferiority complex and other psychological conditions. While clinical inspection followed by confirmatory tests including histopathology and electron microscopic assessment is used for diagnosis, treatment modalities can be further improved for better diagnosis. This article reviews subtypes of ichthyosis, with a focus on EHK, genetics behind the disease, recently reported mutations, the existing diagnostics and treatments for the same and potential of new modalities in diagnosis/treatment.

Meanwhile, the peripheral and central mechanisms of analgesic effect of Liang Mianzhen were further examined via observing the effects of Liang Mianzhen on the signal pathway associated with inflammatory induced hyperalgesia.
