When abdominal pain knocks the door: an unusual presentation of chronic lymphocytic leukemia.

A 67-year-old man presented non-specific abdominal pain. Polypoid mass at appendiceal orifice in the cecum was found on endoscopic investigation without appendicitis sign. Histopathology elucidated underlying mucosal infiltration that was chronic lymphocytic leukemia. This is an isolated and unusual gastrointestinal involvement of hematologic disorder in an older patient.

A Highly Transparent Artificial Photonic Nociceptor.

A nociceptor is an essential element in the human body, alerting us to potential damage from extremes in temperature, pressure, etc. Realizing nociceptive behavior in an electronics device remains a central issue for researchers, designing neuromorphic devices. This study proposes and demonstrates an all-oxide-based highly transparent ultraviolet-triggered artificial nociceptor, which responds in a very similar way to the human eye. The device shows a high transmittance (>65%) and very low absorbance in the visible region. The current-voltage characteristics show loop opening, which is attributed to the charge trapping/detrapping. Further, the ultraviolet-stimuli-induced versatile criteria of a nociceptor such as a threshold, relaxation, allodynia, and hyperalgesia are demonstrated under self-biased condition, providing an energy-efficient approach for the neuromorphic device operation. The reported optically controlled features open a new avenue for the development of transparent optoelectronic nociceptors, artificial eyes, and memory storage applications.

[Peri-/postoperative Antiinflammation Therapy after Cataract and Refractive Surgery].

Cataract surgery is the most frequently performed surgery worldwide, and refractive surgery is gaining more and more popularity. Although they are rare, some possible complications of these procedures remain relevant for surgeons and patients. Macular edema in cataract surgery and sterile infections of the cornea after refractive corneal surgery can lower the visual outcome significantly. Such complications can occur due to activation of inflammation cascades as a result of local tissue manipulation during the surgery. Even though the incidence is very low, there are affected patients as a result of the high number of performed surgeries. Therefore, lowering those numbers with perioperative application of local eye drops is critical. Studies show a lower risk of macular edema, anterior chamber inflammation and postoperative pain in patients treated with perioperative non-steroidal anti-inflammatory drugs (NSAID). That is why a treatment with corticosteroid and NSAID drops should be prescribed. After lamellar corneal surgery, a local steroid application is recommended to lower the incidence of haze and deep lamellar keratitis (DLK).

Intra-medullary osteosclerosis of the tibia is a rare condition characterised by chronic pain due to diaphyseal hyperostosis with no detectable triggering factor. The main differential diagnoses are stress fracture and osteoid osteoma. Of the few cases reported to date, most were in adults. The objective of this study was to assess paediatric patients with intra-medullary osteosclerosis to determine whether the first visit provides sufficient information to establish the diagnosis and rule out both osteoid osteoma and stress fracture, whether a biopsy is required, and which treatment is optimal.

Does the migraine attack start in the cortex and is the cortex critical in the migraine process?

Migraine is a pure human neurovascular disorder with no reliable corresponding animal model. The cerebral cortex (CC) has long been discussed as a crucial element of its complex mechanism. The present review considers the state of the art of experimental evidence on the involvement of CC in migraine. An ample series of research data points to a role of CC in the migraine process, also at the beginning of the attack, especially in the form with aura. However, in spite of several CC, peculiarities emerged in experimental settings, the enigma on their significance (cause or consequence of the disease?) remains. All in all, we believe that pure clinical observations and reasoning, i.e., its exclusive human nature and the precipitating role of stress, are still the most persuasive cues supporting the CC involvement in migraine.
