Evaluation of Risk Factors for Cerebrospinal Leakage in Pediatric Patients With Cerebral Palsy Treated With Intrathecal Baclofen.

Insertion of an intrathecal baclofen (ITB) pump can provide significant benefits in patients with cerebral palsy (CP). However, there are little data describing the risk of complications. Specifically, there is a lack of data describing the incidence of cerebrospinal fluid (CSF) leakage and risk factors following ITB placement. The purpose of our study was to describe risk factors for developing CSF leak in pediatric patients with CP treated with ITB and to report the treatment and outcome of CSF leaks.

Staged treatment for an unusual case of median arcuate ligament syndrome caused by compression of the celiac and superior mesenteric arteries.

We report the unusual presentation of a patient with median arcuate ligament syndrome (MALS) and compression of both the celiac artery (CA) and the superior mesenteric artery (SMA). He underwent a staged treatment. First, a laparoscopic release of the median arcuate ligament was performed. In the second stage, due to persistent post-prandial pain, the SMA was stented resulting in complete symptom relief. Recognizing this rare anatomical presentation is very important to avoiding MALS misdiagnosis and providing the appropriate staged treatment.

Fox-Fordyce Disease: Dermoscopic Perspective.

Fox-Fordyce disease (FFD) is a rare chronic skin disease of the apocrine unit. It presents chiefly in postpubertal females as grouped, monomorphic, skin-colored follicular papules associated with intense pruritus commonly in axillae but may involve pubic, perineal, areolar, and umbilical areas [. 2018;93(1):161-2]. Diagnosis is mostly clinical as histological features are often nonspecific and variable. However, the lesions at times may be mistaken for irritant contact dermatitis, lichen nitidus, syringoma, etc. [ 2009;36(9):485-90]. Dermoscopy is being increasingly utilized in pigmentary and inflammatory skin disorders. In this communication, we describe the dermoscopic features in FFD, which shows typical light brown to dark brown folliculocentric structureless areas with loss of dermatoglyphics. Some of the lesions also show hyperkeratotic follicular plugging.

Role of interventional pain management in patients with chronic pelvic pain.

Patients with chronic pelvic pain are often referred to a variety of specialists in gynecology, urology, and gastroenterology with the hope of finding a diagnosis and treatment. We describe a 51-year-old woman with long-standing chronic pelvic pain secondary to endometriosis who was successfully treated with superior hypogastric plexus blocks. Physicians should consider referring patients to interventional pain specialists for assistance with pain control after thorough diagnostic evaluation.

Impact of quarantine due to COVID infection on migraine: a survey in Genova, Italy.

Quarantine is a well-known risk factor for psychological and psychiatric disturbances. We evaluated burden of migraine during lockdown due to COVID 19 pandemia. Forty-nine subjects followed in our headache clinic for migraine were evaluated for migraine burden by means of global assessment of migraine severity (GAMS) and visual analogue scale (VAS) by phone interview. Moreover, depression and anxiety were quantified by Beck depression inventory (BDI) and Zung Self-Rating Anxiety Scale (SAS). We evaluated changes in the value of migraine score from the 2 months immediately before lockdown (from January 1 to March 9) to the 2 months of quarantine (from March 10 to May 3). Value of GAMS was 5.61 ± 0.76 before and 4.16 ± 1.46 during quarantine (p < .001). VAS was 7.49 ± 1.10 before and 5.47 ± 1.88 during quarantine (p < .001). We also found a time by depression level interaction, F(1,47) = 6.21, p = .016, F(1,47) = 14.52, p < .006, respectively, showing that subjects with lower level of depression had better course of migraine. In conclusion, we showed that, during quarantine due to COVID pandemia, subjects with migraine had fewer migraine attacks and lesser pain and show moderate level of depression, correlated to migraine burden.
