Taking on enduring distress: Building a concept to lead a program of research.

Concept building consists of a 10-phase approach using a practice story as the first step in developing a defined concept supported by both scientific and experiential evidence. The purpose of concept building is to guide development of a program of research that informs nursing knowledge and evidence-based practice. Thus, concept building can serve as a teaching strategy for early-stage doctoral students to initiate scholarly inquiry. In this example, the process of concept building occurred as a nurse practitioner in a doctor of philosophy program began to transition ideas that stemmed from practice into phenomenon for scholarly research. The practice story for the concept taking on enduring distress stems from a patient encounter in the nurse practitioner's adolescent headache clinic. Information gained from concept building laid the foundation for an adolescent headache program of research. The purpose of this paper is to demonstrate a step-by-step approach using Liehr and Smith's 10-phase process for concept building and to describe the program of research that followed.

Baicalin attenuates inflammatory pain associated depressive symptoms via Akt-mediated adult hippocampal neurogenesis.

Depression is one of main symptoms accompanying thermal hyperalgesia and mechanical allodynia induced by inflammatory pain. On physiological level, depressive symptoms could be attenuated by sufficient level of hippocampal neural plasticity. Adult hippocampal neurogenesis (AHN) plays critical roles in clearing panic memory, increasing psychiatric adaptability and preventing depressive emotion. Thus, targeting AHN is the applicable strategy to improve neural functions impaired and attenuate inflammatory pain. Previous reports indicate natural compound baicalin (BA) is one of the effective agents to promote AHN. In present study, we tested the effects of BA in mouse model of inflammatory pain as well as its biological underpinning. Behavioral tests indicate that BA treatment attenuated thermal hyperalgesia, mechanical allodynia and depressive symptoms. Meanwhile, treatment of BA promoted growth and differentiation of neural stem cells in hippocampus. AHN blocker temozolomide (TMZ) resulted in significant suppressed effects of BA to promote AHN, suggesting the critical role of AHN in regulating behavioral effects of BA to inflammatory pain. Akt plays the critical roles in the effects of BA to attenuate inflammatory pain induced symptoms. Prohibiting of Akt with GSK960693 dramatically prevented the effects of BA in attenuating inflammatory pain induced behavioral symptoms. Taken together, BA is the potential pain killer to alleviating inflammatory pain via Akt-mediated adult hippocampal neurogenesis.

Daily acute intermittent hypoxia to improve walking function in persons with subacute spinal cord injury: a randomized clinical trial study protocol.

Restoring community walking remains a highly valued goal for persons recovering from traumatic incomplete spinal cord injury (SCI). Recently, studies report that brief episodes of low-oxygen breathing (acute intermittent hypoxia, AIH) may serve as an effective plasticity-inducing primer that enhances the effects of walking therapy in persons with chronic (> 1 year) SCI. More persistent walking recovery may occur following repetitive (weeks) AIH treatment involving persons with more acute SCI, but this possibility remains unknown. Here we present our clinical trial protocol, designed to examine the distinct influences of repetitive AIH, with and without walking practice, on walking recovery in persons with sub-acute SCI (< 12 months) SCI. Our overarching hypothesis is that daily exposure (10 sessions, 2 weeks) to AIH will enhance walking recovery in ambulatory and non-ambulatory persons with subacute (< 12 months) SCI, presumably by harnessing endogenous mechanisms of plasticity that occur soon after injury.

Headache and Attention Deficit Hyperactivity Disorder (ADHD) are two relatively common, neuropsychiatric conditions seen in children. Recent studies have shown an association between these two disorders, which are otherwise distinct conditions. This study aims to assess the association between migraine and ADHD, as well as the association between screen-time and these two conditions, among children attending a Sri Lankan tertiary care facility. Possible associations will have important implications in the clinical management of these conditions.

[Placebo effects in pain therapy].

In the past few decades, research on pain and placebo analgesia has gained importance both scientifically and clinically. In this article, the current findings and focus of research as well as the significance of placebo research for assessing the effectiveness of pain medication are illustrated. The underlying mechanisms of placebo analgesia not only have implications for theoretical models but also offer clinically relevant guidelines for everyday interventions in pain treatment. However, many placebo phenomena are not fully understood and have to be investigated further in order to exploit the full potential of placebo effects. Interindividual differences and their inclusion in treatment will play a major role in this aspect.
