Clinical Characteristics and Management of Headache: A Real-Life Prospective, Observational Study From a Tertiary Care Center in Eastern India.

To describe clinical profile and management pattern of headache in patients presenting to a tertiary care center.  Methods: In this observational study, demographics, radiological investigations, triggers, and treatment pattern in patients aged ≥ 14 years presenting with headache were recorded. Disability and severity of headache were assessed with Migraine Disability Assessment (MIDAS) score, Visual Analogue Scale (VAS), and Headache Impact Test (HIT-6) in case of migraineurs and VAS and HIT-6 for all other headache disorders. Patients were evaluated at baseline and after three and six months post-treatment.

Uncommon Case of Oral Vancomycin Neurotoxicity With Sexual Dysfunction.

We report a case of a 57-year-old male with a past medical history of controlled hypertension and type 2 diabetes mellitus, who had unusual elevated serum concentration levels of vancomycin during oral treatment for pseudomembranous colitis. Both measured serum vancomycin levels of 39 μg/ml and cerebrospinal fluid level of 5.7 μg/mL were documented and associated with unexplained neurological symptoms and sexual dysfunction. After discontinuing of oral vancomycin and treating with hemodialysis, the patient had reduced serum concentration levels to 26 μg/mL accompanied by significant and rapid resolution of symptoms in addition to the general condition. Patients with intestinal disease and reduced renal function who are being treated with oral vancomycin may absorb and accumulate high serum level amounts of vancomycin, causing uncommon neurological toxication symptoms including headache, altered state of consciousness, confusion, and drowsiness in association with sexual dysfunction.

A narrative review of ultrasound-guided serratus anterior plane block.

Ultrasound-guided serratus anterior plane block (SAPB) is located using ultrasound at the level of the midaxillary line and the fifth rib, and a certain amount of local anesthetics is injected either superficially or deeply into the serratus anterior muscle, blocking the third to sixth intercostal nerves, the long thoracic and thoracodorsal nerves. It is mainly used in breast surgeries, rib fractures and thoracotomy to manage the pain of the anterolateral chest wall. The surgery of anterolateral chest wall is often accompanied by severe postoperative pain, leading to postoperative infection, atelectasis and other complications, and prolonged hospitalization. However, effective pain management can reduce the occurrence of postoperative pulmonary complications, promote patients to get out of bed as soon as possible, and accelerate the recovery of patients. Recently, with the development of ultrasonic technology and equipment, SAPB has entered the era of visualization, further improving the safety and success rate of operations. SAPB, as a new technology of regional block, has a higher positioning accuracy, a higher success rate, lesser complications, and simpler and more effective postoperative analgesia effect. Compared with thoracic epidural analgesia and thoracic paravertebral block, SAPB has a good ultrasonic anatomical basis; thus, has a broad application prospect in clinics. In this paper, the SAPB in clinical application was reviewed.
