Clinicians frequently face questions from headache patients regarding the roles played by sinus issues, muscle tension, and temporomandibular joint (TMJ) problems. This review highlights new concepts regarding the diagnosis and management of these headache conditions and their differentiation from migraine.

Helping the family may either promote or undermine adolescents' physical health and well-being. Adolescents ( = 396, 58% female, = 14.57 years) completed diary checklists for 14 days, reporting whether they provided instrumental assistance (e.g., tangible tasks) and emotional support (e.g., listening, giving advice) to family, as well as their amount of physical activity, sleep, and physical symptoms (e.g., headache, backpain) each day. After providing emotional support, adolescents slept more that night and experienced fewer physical symptoms the next day, over and above prior day levels. When adolescents provided instrumental assistance on school days (but not nonschool days), they engaged in less physical activity that day. These results were consistent across individual differences in gender, age, socioeconomic status, and ethnicity. In addition, bidirectional associations emerged such that adolescents were more likely to provide instrumental assistance on days after they slept more. To present an alternative surgical technique in treating cases of Chiari I Malformation with mild-to-moderate syringomyelia after decompressive suboccipital craniectomy: incising only the outer layer of the dura mater, then dissecting it from the inner layer without opening the latter.

In patients with chronic hepatitis B (CHB) infection, activation of toll-like receptor 8 (TLR8) may induce antiviral immunity and drive functional cure. Selgantolimod, a novel TLR8 agonist, was evaluated in CHB patients who were virally suppressed on oral antiviral treatment (OAV) or viremic not on OAV.

A 79-year old male was admitted to hospital for weight loss and polyarthritis. Examination revealed warmth, erythema and swelling of the left metacarpophalangeal joints, wrist, right shoulder and ankle. The CRP was elevated at 109 mg/L (ULN <5 mg/L) however other rheumatoid markers were within normal limits (ANA, RF, anti-CCP, ANCAs). Radiographs did not demonstrate joint erosions or other changes of inflammatory arthropathy. The patient was started on prednisone, celecoxib and opioids with minimal improvement in pain and function. Paraneoplastic arthritis was suspected.
