Effectiveness of Integrated Education to Reduce Postoperative Nausea, Vomiting, and Dizziness after Abdominal Surgery under General Anesthesia.

This study presents an anticipatory integrated education program for nausea, vomiting, and dizziness prevention (anti-NVD education program) for patients undergoing abdominal surgery under general anesthesia. The anti-NVD education program for nephrectomy patients consisted of the following: the causes of postoperative nausea, vomiting, and dizziness; effective deep breathing and how to use an inspirometer; postoperative nausea and vomiting; effective methods of patient-controlled analgesia; and the stepwise standing up method to prevent dizziness. A study was conducted among 79 adults (experimental group: = 40, control group: = 39). The degree of nausea and dizziness was measured using a numerical rating scale (NRS), and vomiting and the frequency of antiemetic use were measured in terms of the number of patients. The experimental group, which received the anti-NVD education, showed remarkably lower levels of nausea ( = 0.013) and dizziness ( < 0.001) than the control group. The frequency of antiemetic use 48 hours after surgery was significantly lower in the experimental group ( = 0.03). This study proved the efficacy of the anti-NVD education program for reducing postoperative nausea and dizziness. This program can be used as a noninvasive nursing intervention to prevent nausea, vomiting, and dizziness among patients undergoing abdominal surgery.

Infectious Complications of Dorsal Root Ganglion Stimulation: A Systematic Review and Pooled Analysis of Incidence.

Dorsal root ganglion stimulation (DRGS) is a newer form of neuromodulation that targets the dorsal root ganglion. DRGS has superior efficacy in complex regional pain syndrome compared to spinal cord stimulation (SCS) and may have efficacy in other forms of chronic pain. While decades of safety data are available for SCS, there is less available safety information for DRGS. The objectives of this systematic review and pooled analysis of incidence are to determine the overall incidence of DRGS infections, incidence at each stage (trial vs. implant vs. revision), infection characteristics, and outcomes.

Regional analgesia for lower leg trauma and the risk of acute compartment syndrome: Guideline from the Association of Anaesthetists.

Pain resulting from lower leg injuries and consequent surgery can be severe. There is a range of opinion on the use of regional analgesia and its capacity to obscure the symptoms and signs of acute compartment syndrome. We offer a multi-professional, consensus opinion based on an objective review of case reports and case series. The available literature suggested that the use of neuraxial or peripheral regional techniques that result in dense blocks of long duration that significantly exceed the duration of surgery should be avoided. The literature review also suggested that single-shot or continuous peripheral nerve blocks using lower concentrations of local anaesthetic drugs without adjuncts are not associated with delays in diagnosis provided post-injury and postoperative surveillance is appropriate and effective. Post-injury and postoperative ward observations and surveillance should be able to identify the signs and symptoms of acute compartment syndrome. These observations should be made at set frequencies by healthcare staff trained in the pathology and recognition of acute compartment syndrome. The use of objective scoring charts is recommended by the Working Party. Where possible, patients at risk of acute compartment syndrome should be given a full explanation of the choice of analgesic techniques and should provide verbal consent to their chosen technique, which should be documented. Although the patient has the right to refuse any form of treatment, such as the analgesic technique offered or the surgical procedure proposed, neither the surgeon nor the anaesthetist has the right to veto a treatment recommended by the other.

Brain arteriovenous malformations and dural arteriovenous fistulas with extensive venous congestive encephalopathy.

In brain arteriovenous malformations (BAVMs) and dural arteriovenous fistulas (DAVFs), when too much blood is drained into the venous system, extensive venous congestive encephalopathy (EVCE) can appear. EVCE in BAVMs and DAVFs can be divided into acute and chronic stages. BAVMs and DAVFs have their own classification systems, but EVCE is not considered in these classification systems and needs to be emphasized. EVCE in BAVMs and DAVFs has unique clinical and imaging features. The clinical presentations usually consist of headache, cognitive impairment, and focal deficits. EVCE in BAVMs and DAVFs has several imaging features, and the venous congestion seen on computed tomography angiography and magnetic resonance angiography can present with the angiographic features of venous reflux and pseudophlebitic pattern. Digital subtraction angiography is the gold standard for the diagnosis. Delayed circulation time is observed. Tortuous, dilated, and engorged veins can be seen. For EVCE from BAVMs and DAVFs, prompt treatment is warranted due to the impairment of extensive brain tissue. Treatments include endovascular treatment (EVT), open surgery, and radiosurgery. EVT is often the primary treatment. Complete elimination in one stage is often difficult. Most of the time, staged treatment has to be chosen. No matter at the acute or chronic stage, aggressive treatment is recommended.
